통합돌봄 100일의 성과와 과제

지역사회 통합돌봄 제도가 시행 100일을 맞아 전국에서 37천여 명이 맞춤형 돌봄 서비스를 받았다. 통합돌봄의 지역 간 서비스 격차와 낮은 정책 인지도, 의료·돌봄 인프라 부족 등은 개선해야 할 과제로 드러났다.

<지역사회 통합돌봄은 신청해야 받는다>

지역사회 통합돌봄은 일상생활에 어려움을 겪는 사람이 살던 곳에서 건강한 생활을 영위하도록 의료·요양·돌봄서비스를 통합·연계하여 제공하는 제도이다. 관련 법률과 지방자치단체의 조례에 따라 올해 327일부터 시행되고 있다.

주된 대상자는 노쇠·장애·질병 등으로 일상생활 유지에 어려움 있어 복합적 지원이 필요한 노인, 장애인, 지방자치단체장이 필요성을 인정하는 사람이다. 당사자나 가족 등이 읍동 행정복지센터에 신청하면 지자체(공무원)가 선정하여 서비스를 연계하여 지원한다.

통합돌봄은 돌봄이 필요한 노인과 장애인 등이 (요양)병원이나 요양시설 등에 의존하지 않고 일상생활을 유지할 수 있도록 돕는 것이 핵심이다. 대표적인 지원 대상은 고령으로 일상생활에 도움이 필요한 사람, 병원에서 퇴원 후 돌봄이 필요한 사람, 장기요양 또는 복지서비스 연계가 필요한 사람, 장애나 질병으로 생활 지원이 필요한 사람, 가족 돌봄 부담이 큰 가구 등이다. 혼자 생활하거나 보호자의 돌봄이 어려운 경우 우선적으로 지원받을 수 있다.

신청자들은 더러 현금 지원을 기대하지만 서비스 지원이 중심이다. 대표적인 서비스는 방문 돌봄, 식사 지원, 건강관리, 병원 동행, 재활 지원 등이다. 상황에 따라 주거 환경 개선이나 생활 지원이 함께 제공된다. 가족이 직접 간병해야 하는 시간을 줄일 수 있어서 체감 효과가 큰 편이다.

 

<돌봄통합지원법 시행 100일의 성과를 점검했다>

보건복지부는 돌봄통합지원법 시행 100일을 맞아 운영성과를 점검했다. 지난 626일 기준 통합돌봄 신청자는 46215명이고, 그중 37304명이 서비스를 연계받았다고 밝혔다. 이용자 1인당 평균 3.3건의 서비스를 지원받았으며, 총 서비스 제공 건수는 123595건에 달했다.

서비스는 가사지원, 이동지원 등 일상생활돌봄이 43.1%로 가장 많았고, 건강관리예방, 장기요양, 주거복지, 보건의료 등이 뒤를 이었다. 특히 지방자치단체가 지역 특성에 맞춰 자체 개발한 지역특화 서비스가 전체의 37.4%를 차지하며 지역 중심 돌봄체계가 점차 자리 잡고 있는 것으로 나타났다.

 

<돌봄 사각지대를 해소하고 있다>

지역사회 통합돌봄을 통해 돌봄 사각지대를 해소한 사례도 많았다. 암 수술 이후 돌봄 공백에 놓인 고령자에게 가사·식사 지원과 방문건강관리, 주거환경 개선 등을 통합 제공했다는 사례가 대표적이다. 시각장애와 구강암을 앓는 한 독거노인에게 방문진료와 활동지원, 응급안전서비스를 연계해 지역사회 생활을 이어갈 수 있도록 지원한 사례가 있었다.

또한, 퇴원환자와 중증장애 배우자를 함께 지원하거나 중간집을 활용해 퇴원 후 지역사회 복귀를 돕는 등 가족 단위와 퇴원환자 중심의 통합돌봄 모델도 성과를 보였다. 중간집은 병원 퇴원 후 일상 복귀까지 최대 3개월 동안 머물며 집중적인 맞춤형 돌봄서비스를 지원받을 수 있는 주거공간이다.

 

<통합돌봄이 가족의 돌봄 부담을 줄여준다>

국민 인식조사에서는 지역사회 통합돌봄 제도에 대한 기대감도 확인됐다. 응답자의 94.7%는 통합돌봄이 가족의 돌봄 부담을 줄이는 데 기여할 것으로 전망했고, 93.8%는 본인이 돌봄이 필요할 경우 이용할 의향이 있다고 답했다.

하지만, 실제 제도 시행 사실을 알고 있다는 응답은 57.1%에 그쳐 정책 홍보 강화가 시급한 과제로 지적됐다. 또 가장 도움이 될 서비스로는 일상생활돌봄을 꼽았으며, 앞으로 방문재활과 이동·병원동행, 재택 임종돌봄 등에 대한 수요가 높은 것으로 조사됐다.

 

<현장 의견을 반영해 개선할 계획이다>

정부는 시행 과정에서 드러난 현장 의견을 반영해 제도개선에 나설 계획이다. 현재 방문 신청 중심인 신청 체계를 개선하기 위해 2027년 통합돌봄 전산시스템에 온라인 신청 기능을 도입하고, 방문재활과 방문영양, 재가임종 등 신규 서비스 모델을 개발해 단계적으로 제도화할 예정이다.

의료취약지와 인구감소지역에는 지역특화 서비스 예산을 차등 지원해 지역 간 서비스 격차를 줄이고, 지방정부의 성과를 이용자 만족도와 재가생활 유지기간 등 성과 중심으로 평가해 예산과 연계하는 방안도 추진한다. 아울러 76일부터 20일까지 지방자치단체를 대상으로 제도 개선 건의를 집중 접수해 현장의 목소리를 정책에 반영할 계획이다.

 

<국민이 체감할 수 있는 정책을 만든다>

최근 보건복지부는 1차 지역사회 통합돌봄 포럼을 개최했다. 가천대 남현주 교수는 돌봄 기본 권리 시대의 통합돌봄: 세계 동향과 한국 정책 방향이란 주제로 돌봄을 사회 권리로 다시 세우는 세계적인 흐름을 소개하고 제1차 지역사회 통합돌봄 기본계획의 방향을 제안했다.

국민건강보험공단 통합돌봄정책개발센터 유애정 센터장은 통합돌봄의 현재, 나아가야 할 길이란 주제로 시범 사업부터 전국으로 확대된 과정과 함께 앞으로 대상자를 늘리고 서비스를 다시 짜는 과제들을 제시했다.

이어서 전문가들은 통합돌봄이 현장에 잘 자리 잡고 발전할 수 있는 방법들을 논의했다. 보건복지부 은성호 실장은 시행 100일을 맞이하여 제도가 현장에 깊이 뿌리내릴 때라고 강조하고, 포럼에서 나온 이야기들을 제1차 기본계획에 반영시킬 것을 약속했다. 이번 포럼은 돌봄 기본 권리 시대를 맞아 지역사회에서 통합적으로 돌봄을 제공하는 방안을 모색하는 자리였으며, 특히 국민이 체감할 수 있는 정책을 만드는 데 초점을 맞췄다.

 

<통합돌봄 인력 충원 없이 업무가 늘었다>

민주노총 공공운수노조는 건강보험공단 소속 통합돌봄 담당자와 지자체·보건소 방문간호사 등 조합원 232명을 대상으로 통합돌봄 시행 이후 현장 실태를 조사했다. 응답자 중 83%인력 충원 없이 업무가 늘었다고 답했다. 현재 인력으로 통합돌봄 업무를 수행하기 충분한지를 묻자 84.6%부족하다고 답하고, ‘충분하다는 답변은 4.3%에 그쳤다.

늘어난 업무는 방문상담·가정방문이 63.8%로 가장 많았고, 이어서 회의 참석(54.8%), 민원 응대(49.5%), 대상자 발굴(47.9%), 전산 입력(47.9%) 등의 순이었다. 통합돌봄이 시행된 후 응답자의 64.4%통합돌봄 시행 후 민원이나 감정노동이 증가했다고 답했으며, ‘심리적 부담이 커졌다는 답변도 77.1%에 달했다.

통합돌봄 담당자 등이 생각하는 최우선 개선 과제로는 인력 충원이 70.7%이 가장 높고, 그 다음은 전담인력 배치(45.7%), 통합돌봄 관련 조직 개편과 통합적 운영(37.8%), 기관 간 책임과 역할 명확화(37.2%) 등이 뒤를 이었다. 충원이 가장 필요한 인력은 방문서비스 인력(46.3%)이고, 그 다음은 의료·간호 연계 인력(13.3%), 현장 돌봄 인력(13.3%), 사례관리 인력(9%), 행정·전산 입력 인력(6.4%) 순으로 나타났다. 조사결과를 바탕으로 공공운수노조는 통합돌봄이 현장 노동자의 희생에 의존하는 방식으로 운영돼서는 안 된다, 담당자들의 의견을 적극 반영하여 개선할 것을 요구했다.

100 Days of Integrated Care and Tasks

On the 100th day of the implementation of the integrated community care system, more than 37,000 people across the country received customized care services. The service gap between regions of integrated care, low policy awareness, and lack of medical and care infrastructure turned out to be tasks to be improved.

 

Integrated community care is a system that integrates and links medical, care, and care services so that people who have difficulties in their daily lives can lead a healthy life in the place where they lived. It has been in effect since March 27 of this year in accordance with related laws and ordinances of local governments.

The main target is a person who recognizes the necessity of the elderly, persons with disabilities, and heads of local governments who need complex support due to difficulties in maintaining daily life due to old age, disability, disease, etc. When a party or family member applies to the Eup/Myeon/Dong Administrative Welfare Center, the local government (public official) selects and supports the service in connection.

The key to integrated care is to help the elderly and the disabled in need of care maintain their daily lives without relying on (care) hospitals or nursing facilities. Representative support targets include those who are elderly and need help with daily life, those who need care after being discharged from the hospital, those who need long-term care or welfare service linkage, those who need life support due to disability or disease, and households with a high burden of family care. If you live alone or have difficulty caring for your guardian, you can receive support first.

Applicants sometimes expect cash support, but service support is centered. Typical services include visiting care, meal support, health care, hospital accompanying, and rehabilitation support. Depending on the situation, residential environment improvement or living support is provided. It is very effective because it can reduce the time the family has to take care of themselves.

 

The Ministry of Health and Welfare checked the operational performance on the 100th day of the enforcement of the Integrated Care Support Act. As of June 26, 46,215 applicants for integrated care were reported, of which 37,304 were linked to the service. Each user received an average of 3.3 services, and the total number of services provided reached 123,595.

As for the service, daily life care such as domestic support and mobile support was the most at 43.1%, followed by health care prevention, long-term care, residential welfare, and health care. In particular, regional specialized services developed by local governments according to regional characteristics accounted for 37.4% of the total, and the regional-centered care system was gradually taking place.

 

There were also many cases in which the blind spots of care were resolved through integrated care in the community. A typical case is that housekeeping and meal support, visiting health management, and residential environment improvement were integrated and provided to the elderly who were in a care gap after cancer surgery. There was a case of supporting an elderly person living alone with a visual impairment and oral cancer to continue living in the community by linking visiting medical care, activity support, and emergency safety services.

In addition, the integrated care model centered on family units and discharged patients also showed results, such as supporting discharged patients and spouses with severe disabilities together or using an "intermediate house" to help them return to the community after discharge. The middle house is a residential space where you can stay for up to three months from hospital discharge to daily return and receive intensive customized care services.

 

The public perception survey also confirmed expectations for the integrated care system in the community. 94.7% of the respondents predicted that integrated care would contribute to reducing the burden of care on the family, and 93.8% said they would be willing to use it if they needed care.

However, only 57.1% of the respondents said they were aware of the actual implementation of the system, pointing out that strengthening policy promotion is an urgent task. In addition, daily life care was cited as the most helpful service, and it was found that there is a high demand for visiting rehabilitation, mobile and hospital companionship, and end-of-life care at home.

 

The government plans to improve the system by reflecting on-site opinions revealed during the implementation process. In order to improve the application system, which is currently centered on visiting applications, the online application function will be introduced into the integrated care computer system in 2027, and new service models such as visiting rehabilitation, visiting nutrition, and home-based pregnancy will be developed and institutionalized in stages.

The government plans to narrow the service gap between regions by providing differential support for regional specialized service budgets to medical vulnerable areas and population-reduced areas, and to link local government's performance with the budget by evaluating the performance based on user satisfaction and the period of maintenance of home life. In addition, it plans to receive proposals for system improvement from local governments from July 6 to 20 to reflect the voices of the field in its policies.

 

Recently, the Ministry of Health and Welfare held the 1st Community Integrated Care Forum. Professor Nam Hyun-joo of Gachon University introduced the global trend of re-establishing care as a social right under the theme of 'Integrated Care in the Age of Basic Care Rights: World Trends and Policy Direction in Korea' and suggested the direction of the 1st Community Integrated Care Basic Plan.

Yoo Ae-jeong, head of the National Health Insurance Corporation's Integrated Care Policy Development Center, presented tasks to increase the number of targets and reorganize services in the future, along with the process of expanding from pilot projects to the country under the theme of "The Current, Way to Go."

Experts then discussed ways for integrated care to be well-established and developed in the field. Eun Sung-ho, head of the Ministry of Health and Welfare, stressed that it is time for the system to be deeply rooted in the field on the 100th day of implementation and promised to reflect the stories from the forum in the first basic plan. This forum was a place to seek ways to provide integrated care in the community in the era of basic rights to care, especially focusing on creating policies that can be felt by the public.

 

The public transport union of the Korean Confederation of Trade Unions surveyed 232 union members, including integrated care managers belonging to the Health Insurance Corporation and visiting nurses from local governments and health centers, on the site after the implementation of integrated care. Of the respondents, 83% said, "Work has increased without additional manpower." When asked if the current workforce is sufficient to carry out integrated care work, 84.6% said it was "insufficient," and only 4.3% said it was "enough."

Visiting counseling and home visits were the most common tasks at 63.8%, followed by meeting (54.8%), responding to civil complaints (49.5%), finding targets (47.9%), and computer input (47.9%). After the integrated care was implemented, 64.4% of the respondents said "civil complaints or emotional labor increased after the integrated care was implemented," and 77.1% said "the psychological burden increased."

The highest priority improvement task considered by integrated care managers was the recruitment of 70.7%, followed by the deployment of dedicated personnel (45.7%), organizational reorganization and integrated operation related to integrated care (37.8%), and clarification of responsibilities and roles between institutions (37.2%). The most needed manpower is the visiting service manpower (46.3%), followed by medical and nursing-related personnel (13.3%), on-site care personnel (13.3%), case management personnel (9%), and administrative and computer input personnel (6.4%). Based on the results of the survey, the Public Transport Workers' Union said, "Integrated care should not be operated in a way that relies on the sacrifice of field workers," and called for improvement by actively reflecting the opinions of those in charge.
