국민의 5.5%가 기초생활보장 수급자이다

2025년 국민기초생활보장 수급자가 약 284만명이고 전체 국민의 5.5%이었다. 일반수급자의 44.4%65세 이상 노인이고, 전체 노인 중 기초생활보장 수급자는 11.2%이었다.

 

<정부가 기초생활보장 수급자 현황을 발표했다>

보건복지부가 공개한 ‘2025년 국민기초생활보장 수급자 현황에 따르면 지난해 12월 말 기준 수급자는 2836706(2117790가구)으로, 전년(2673485)보다 6.1% 늘었다. 이 가운데 일반수급자가 96.3%2732235, 사회복지시설 등에 거주하는 시설수급자가 3.7%104471명이다.

기초생활보장제도는 저소득층의 생활을 돕기 위해 국가가 급여를 지급하는 복지제도로, 가구 소득인정액이 기준중위소득의 일정 비율 이하이면서 부양할 사람이 없거나 있어도 부양 능력이 없어야 대상이 된다. 기초생활보장 수급자는 크게 생계급여, 의료급여, 주거급여, 교육급여 수급자로 나뉜다.

 

<생계급여 수급자는 185만명이었다>

생계급여 수급자는 가구 소득인정액이 기준중위소득의 32% 이하이고 부양의무자가 없거나 부양능력이 낮을 때 선정되어 생계급여를 받는다. 생계급여 수급자는 185688명이고 전체 국민의 3.6%이다. 생계급여 수급자는 매월 정부가 정한 생계급여 기준에서 가구소득인정액을 감하고 나머지 금액을 현금으로 받을 수 있다.

매월 받을 수 있는 최대액은 1인 가구 820,556, 2인 가구 1,343,773, 3인 가구 1,714,892, 4인 가구 2,078,316원이다. 해당 가구에 소득인정액이 30만 원이 있다면, 그만큼 공제하고 받을 수 있다. 생계급여 수급자중 독거노인이 기초연금 349,700원을 받으면 다음 달 생계급여에서 그것을 뺀 470,856원까지 받을 수 있다.

 

<의료급여 수급자는 부양의무자 기준이 엄격하다>

의료급여 수급자는 가구 소득인정액이 기준중위소득의 40% 이하이고 부양의무자가 없거나 부양능력이 낮을 때 선정되어 의료급여를 받는다. 의료급여 수급자는 1547174명이고 전체 국민의 3.0%이다. 소득인정액이 기준중위소득의 40% 이하일 때 선정되는 의료급여 수급자가 생계급여 수급자보다 303514명이 적은 것은 부양의무자의 부양능력을 산정하는 방식이 다르기 때문이다.

부양의무자는 1촌 이내의 직계가족(자녀 혹은 부모)과 그 배우자라는 점은 같지만, 생계급여 수급자는 부양의무자의 가구 소득이 연간 13천만원 초과 혹은 재산이 12억원 초과인 경우에만 부양능력이 있는 것으로 본다. 하지만, 의료급여 수급자를 선정할 때에는 부양의무자의 소득인정액이 기준중위소득의 140%를 넘으면 엄격하게 산정한다. 노인에게 어느 정도 생활하는 자녀가 있거나, 청년에게 부모가 있으면 의료급여 수급자로 선정되기 어렵다.

의료급여 수급자는 가구 구성원이 18세 미만, 65세 이상, 장애인만으로 구성되어 있으면 1종 서비스를 받고, 18세 이상 65세 미만인 사람이 한 명만 있어도 2종 서비스를 받는다. 1종은 병의원과 약국을 이용할 때 사실상 본인부담금이 거의 없고, 2종은 본인부담금이 진료비의 10~15%이다.

 

<주거급여 수급자는 부양의무자 기준이 없다>

가구 소득인정액이 기준중위소득의 48% 이하인 경우에 선정되는 주거급여 수급자는 2663802명이고 전체 국민의 5.2%로 가장 많다. 주거급여 수급자는 거주지역과 가구원수에 따라 기준 임대료의 범위에서 월세를 지원받고, 전세로 사는 사람도 월세로 환산한 금액까지 지원받을 수 있다. 생계급여, 의료급여, 주거급여 수급자 등은 공공임대주택을 신청하면 우선적으로 선정될 수 있다.

주거급여는 학업이나 취업 등을 이유로 두 집 살림을 하고 신청하면 각각 받을 수도 있다. 주거급여는 부양의무자 기준이 없기에 부모와 따로 사는 청년이 신청하면 수급자로 선정될 가능성이 높다.

 

<교육급여 수급자는 대학생 국가장학금을 활용할 수 있다>

가구 소득인정액이 기준중위소득의 50% 이하일 때 선정되는 교육급여 수급자는 306577명으로 전체 인구의 0.6%이었다. 교육급여 수급자는 초고등학교에 재학중일 때만 선정될 수 있기에 그 수가 많지 않았다.

교육급여 수급자는 소정의 교육활동비를 바우처로 받을 수 있다. 자립형 사립고등학교와 같이 등록금을 내야 하는 고등학생은 소정의 등록금 전액을 지원받을 수 있다.

교육급여 수급자는 대학교에 입학할 때 특례입학 대상자이고, 입학한 후에는 최대 8학기까지 국가장학금을 받을 수 있다. 일반 학생은 전 학기 성적이 평점 B학점(100점 만점에 80) 이상일 때 국가장학금을 받을 수 있지만, 기초생활보장 수급자는 C학점(70) 이상일 때 받을 수 있다. 65세 미만인 조건부 수급자는 대학에 다니면서 노동능력을 키우는 것이 중장기적으로 유리할 수 있다. 한부모가족은 자녀가 성장하면 소득인정액이 높아져서 수급자로부터 탈락되는 경우가 많다. 자녀 혹은 부모가 대학교에 진학하면 학생은 근로의무가 없기에 생계급여를 받으면서 노동능력을 키울 수도 있다.

 

<수급자는 경기지역에 가장 많이 산다>

국민기초생활보장 수급자의 거주지역을 보면, 경기지역이 548929(38151가구)으로 전국 17개 시·도 가운데 가장 많았다. 그 다음으로 많이 사는 서울(465567)보다 8만명 이상 많다. 인천은 198136(134755가구)으로 집계됐다. 기초생활보장 수급자도 수도권에 전체의 42.7%(1212632)가 살고 있다. 다만 인구 대비 수급자 비율인 수급률은 경기가 4.0%로 세종(2.6%)에 이어 전국에서 두 번째로 낮았다. 인천은 6.5%로 전국 평균(5.5%)을 웃돌았고, 서울은 5.0%였다. 수급률이 가장 높은 곳은 전북(8.2%)이었고 부산 7.9%, 광주 7.6% 순이었다.

 

<일반 수급자의 44.4%는 노인이다>

국민기초생활보장 수급자의 연령은 고령층 쏠림이 두드러졌다. 일반수급자 2732235명 가운데 65세 이상이 1212262명으로 44.4%를 차지했다. 이어 중년(40~64) 31.7%, 청년(20~39) 9.9%, 청소년(12~19) 8.8%, 학령기(6~11) 4.0%, 영유아(0~5) 1.2% 순이었다.

총인구 대비 일반수급자 비율은 5.3%(시설수급자 포함 시 5.5%)였는데, 65세 이상 중에서는 이 비율이 11.2%로 가장 높았다. 노인 10명 중 1명 이상이 기초생활보장 수급자인 셈이다. 성별로는 여성 수급자가 1517149(55.5%)으로 남성 1215086명보다 많았다.

 

<일반 수급자의 74.7%1인가구였다>

가구 특성에서도 고령·1인 가구 비중이 높았다. 일반수급 2013319가구 중 1인 가구가 1504789가구로 74.7%를 차지했으며 2인 가구 15.6%, 3인 가구 5.8% 순이었다. 가구유형별로는 노인가구가 30.0%로 가장 많았고 장애인가구 11.5%, 모자가구 10.5%, 부자가구 2.8% 등 취약계층 가구가 전체의 55%가량을 차지했다. 보장기간이 10년 이상인 가구도 19.9%에 달해, 장기 수급 상태가 고착화된 양상을 보였다.

5.5% of the population are recipients of the Basic Livelihood Security

In 2025, there were about 2.84 million recipients of the National Basic Livelihood Security, accounting for 5.5% of the total population. 44.4% of general recipients are elderly people aged 65 or older, and 11.2% of all elderly people received basic livelihood security benefits.

 

According to the Ministry of Health and Welfare’s “2025 Status of National Basic Livelihood Security Recipients,” as of the end of December last year, there were 2,836,706 recipients (2,117,790 households), an increase of 6.1% from the previous year (2,673,485). Among them, 2,732,235 people, or 96.3%, are general recipients, while 104,471 people, or 3.7%, are facility recipients living in social welfare facilities and similar institutions.

The Basic Livelihood Security System is a welfare program in which the state provides benefits to help low-income households improve their living conditions. Eligibility requires that the household’s recognized income is at or below a certain percentage of the median income, and that there are no dependents or, if there are dependents, that they are unable to support them. Recipients of the Basic Livelihood Security are broadly divided into recipients of livelihood benefits, medical benefits, housing benefits, and education benefits.

There were 1.85 million recipients of livelihood benefits

Livelihood benefit recipients are selected and receive livelihood benefits when the household’s recognized income is 32% or less of the median income, and there is no obligated supporter or the supporter has low ability to support the household. There are 1,850,688 recipients of livelihood benefits, which is 3.6% of the total population. Recipients of livelihood benefits can receive cash each month after deducting the household’s recognized income from the government-set livelihood benefit standard, and the remaining amount.

The maximum monthly amount is 820,556 won for a single-person household, 1,343,773 won for a two-person household, 1,714,892 won for a three-person household, and 2,078,316 won for a four-person household. If the household’s recognized income is 300,000 won, they can receive the amount after deducting that. Among livelihood benefit recipients, if an elderly person living alone receives a basic pension of 349,700 won, they can receive up to 470,856 won next month after deducting that amount from the livelihood benefit.

 

Medical aid recipients are selected and receive medical aid when the household’s recognized income is 40% or less of the median income, and there is no obligated supporter or the supporter has low ability to support the household. There are 1,547,174 medical aid recipients, which is 3.0% of the total population. The reason there are 303,514 fewer recipients of medical benefits selected when the recognized income amount is 40% or less of the standard median income than recipients of livelihood benefits is that the method for determining the obligated supporter’s ability to provide support differs.

A person obligated to support is the same as a direct family member within the first degree of kinship (child or parent) and their spouse, but a recipient of livelihood benefits is considered capable of supporting only if the obligated supporter’s household income exceeds 130 million won annually or their assets exceed 1.2 billion won. However, when selecting recipients of medical benefits, if the recognized income of the obligated supporter exceeds 140% of the standard median income, it is calculated strictly. It is difficult to be selected as a recipient of medical benefits if the elderly person has children of a certain level of ability to support themselves, or if the young adult has parents.

Medical aid recipients receive Type 1 services if their household consists only of members under 18, 65 or older, or disabled, and they receive Type 2 services if there is even one person aged 18 or older but under 65. Type 1 means that when using hospitals, clinics, and pharmacies, there is virtually no out-of-pocket expense, while Type 2 means the out-of-pocket expense is only 1015% of the medical cost.

 

There are 2,663,802 recipients of housing benefits selected when the household’s recognized income is 48% or less of the median income, making up 5.2% of the total populationthe largest group. Housing benefit recipients can receive monthly rent support within the standard rent limit depending on their place of residence and household size, and those living under a jeonse contract can also receive support up to the amount converted to monthly rent. Recipients of livelihood benefits, medical benefits, and housing benefits, among others, may be given priority in selection when applying for public rental housing.

With housing benefits, if you live in two households for reasons such as education or employment and apply, you may receive benefits for each. Since there is no support obligation standard for housing benefits, young people living separately from their parents are likely to be selected as recipients if they apply.

 

The number of education benefit recipients selected when the household recognized income is 50% or less of the median income was 306,577, accounting for 0.6% of the total population. Since recipients of educational benefits could only be selected while enrolled in elementary, middle, or high school, the number was not large.

Recipients of educational benefits may receive a predetermined educational activity allowance in the form of a voucher. High school students who must pay tuition, such as those attending autonomous private high schools, can receive full support for the tuition fees.

Recipients of educational benefits are eligible for special admission when entering university, and after enrollment, they can receive the national scholarship for up to eight semesters. Regular students may receive the national scholarship if their overall grade for the entire semester is a B grade or higher (80 out of 100), whereas recipients of the Basic Livelihood Security can receive it if their grade is a C grade or higher (70 out of 100). For “conditional recipients” under the age of 65, it may be advantageous in the medium to long term to attend university while developing their ability to work. In single-parent families, as the children grow, the recognized income amount increases, and they often lose eligibility for benefits. If a child or parent goes to university, the student has no obligation to work, so they can develop their ability to work while receiving livelihood benefits.

 

Looking at the residential areas of recipients of the National Basic Livelihood Security, Gyeonggi Province had the largest number among the 17 metropolitan and provincial areas nationwide, with 548,929 people (380,151 households) in the Gyeonggi region. It is more than 80,000 more than Seoul, which has the second-highest number of buyers next (465,567). Incheon was counted at 198,136 people (134,755 households). Basic livelihood security recipients also live in the metropolitan area, accounting for 42.7% of the total (1,212,632 people). However, the recipient ratethe ratio of recipients to the populationwas 4.0% in Gyeonggi, the second lowest in the country after Sejong (2.6%). Incheon exceeded the national average of 5.5% with 6.5%, and Seoul was 5.0%. The region with the highest supply-demand ratio was North Jeolla Province (8.2%), followed by Busan at 7.9% and Gwangju at 7.6%.

 

<44.4% of general recipients are elderly people>

The age distribution among recipients of the National Basic Livelihood Security showed a pronounced concentration among the elderly. Among the 2,732,235 general beneficiaries, 1,212,262 were aged 65 or older, accounting for 44.4%. It was followed by middle-aged (4064 years old) at 31.7%, young adults (2039 years old) at 9.9%, adolescents (1219 years old) at 8.8%, school-age children (611 years old) at 4.0%, and infants and young children (05 years old) at 1.2%.

The proportion of general beneficiaries relative to the total population was 5.3% (5.5% including facility beneficiaries), and among those aged 65 and older, this ratio was the highest at 11.2%. That means more than one in ten elderly people receive basic livelihood security benefits. By gender, there were 1,517,149 female recipients (55.5%), which was more than the 1,215,086 male recipients.

 

<74.7% of general recipients were single-person households>

The proportion of elderly and single-person households was also high in terms of household characteristics. Among the 2,013,319 general households, single-person households accounted for 1,504,789 households, or 74.7%, followed by two-person households at 15.6%, and three-person households at 5.8%. By household type, elderly households accounted for the largest share at 30.0%, followed by households with disabled members at 11.5%, single-mother households at 10.5%, and wealthy households at 2.8%, with vulnerable households making up about 55% of the total. Households with a coverage period of 10 years or more also reached 19.9%, showing a pattern in which long-term benefit recipients have become entrenched.
