발달장애인 돌봄의 국가책임을 강화한다

정부는 발달장애인이 부모 없이도 안전하게 살 수 있도록 발달장애인의 국가책임 돌봄을 강화한다고 밝혔다.

 

<성인 발달장애인의 돌봄을 어떻게 할 것인가>

중증 자폐아들을 ‘피터팬’이라 불렀던 고 전경철 작가가 죽음을 앞두고 마지막까지 걱정했던 것은 자신이 떠난 뒤 홀로 남을 아들의 돌봄이었다. 그가 자녀를 위해 1000여 곳의 장애인복지시설을 찾아갔으나 거절당한 사연이 알려지면서 사회적 관심이 더욱 커졌다.

발달장애인의 경우 인지 능력이 상대적으로 저조하고 의사소통이 어려운 경우가 많아 부모 등 주 보호자 부재시 돌봄과 생존의 공백에 직면하게 된다. 발달장애 자녀를 둔 부모들은 죽은 후에는 누가 돌볼 것인지를 걱정하여, 자녀보다 하루 더 사는 것을 희망한다.

많은 나라에서 아동・청소년의 돌봄과 교육은 18세 쯤 끝난다. 그런데, 발달장애인의 돌봄은 성인이 된다고 끝나지 않는다. 정부는 발달장애인의 돌봄 공백을 줄이기 위해 영유아기부터 성인까지 생애단계별 지원을 강화하기로 했다.

 

<발달장애인 돌봄의 국가책임제를 발표했다>

보건복지부는 관계부처 합동으로 국정과제의 하나인 ‘발달장애인 돌봄 국가책임제’ 추진방안을 발표했다. 지적·자폐성 장애인 등 발달장애인은 인지·의사소통의 제약과 자해·타해 같은 도전행동으로 일상적인 지원이 필요하다. 스스로 일상생활을 할 수 있는 비율이 전체 장애인은 64.8%인데, 발달장애인은 26.4%에 불과하다.

발달장애인은 2020년 24만8천명에서 2025년 28만8천명으로 늘었고, 전체 장애인에서 차지하는 비율도 9.4%에서 11.0%로 높아졌다. 발달장애인의 연령은 2025년에 성인(18∼64세)이 69.5%이고, 65세 이상이 6.0%이었다. 이는 우리나라 전체 등록장애인 중 65세 이상이 2020년 49.9%에서 2025년 56.9%로 늘어 고령화한 것과 다르다. 발달장애인의 사망 당시 평균 연령은 57.2세로, 전체 장애인 사망 당시 평균 연령(78.3세)과 21.1년이나 낮다.

따라서 발달장애인에 대한 국가의 지원은 짧은 생애동안 집중적으로 제공되어야 한다. 생활수준과 의료의 발달로 발달장애인의 수명도 점차 늘어나기에 중장년에 대한 관심을 키워야 한다. 정부는 최근 5년간 연평균 증가율(약 3.1%)을 고려했을 때 2030년에 발달장애인이 33만5천명(전체 대비 12.8%)으로 늘 것으로 내다봤다. 2025년 기준 성인의 비율을 적용하면 2030년 성인 발달장애인은 23만3천명가량으로 늘어날 것이다.

 

<장애아동의 양육지원 시간을 늘린다>

정부는 발달장애인 돌봄 국가책임제를 통해 장애인이 자립할 수 있도록 지원하는 방안을 다양하게 마련했다. 복지부 소관 발달장애인 지원 예산은 2015년 40억원에서 2026년 4천880억원으로 약 122배가 된 데 이어 2027년 정부안에는 5천721억원이 편성됐다. 2014년에 ‘발달장애인 권리보장 및 지원에 관한 법률’이 제정되면서 관련 예산이 크게 늘었다.

정은경 보건복지부 장관은 “2027년 범부처의 관련 예산은 2조8천억원 정도로 15.9% 증액하도록 반영한 상태”라며 “2030년이 되면 발달장애인이 더 늘어나기 때문에 그에 맞는 서비스양의 확보가 필요할 텐데, 매년 예산을 확보하겠다”고 말했다.

정부는 우선 양육 부담을 덜도록 장애아동 가족의 양육 지원 이용 시간을 2026년 연 1천200시간에서 2027년 1천320시간으로 10% 늘린다. 현재 2천8곳인 장애아 전문·통합 어린이집도 내년에는 80곳을 추가한다. 장애아동 조기 개입 등을 위해 발달재활 서비스 대상자는 올해 11만명에서 내년 11만6천명으로, 방과후활동서비스 대상자는 같은 기간 1만1천500명에서 1만3천명으로 늘린다. 장애 아동에 대한 사회적 지원 시간과 예산을 늘리면 부모 등의 돌봄 부담을 줄일 수 있을 것이다.

 

<성인 발달장애인을 위해 주간 활동 서비스를 늘린다>

정부는 성인 발달장애인을 위해서는 주간 활동 서비스 대상자를 현재 1만5천명에서 내년 2만명, 2030년 3만명으로 확대할 계획이다. 정부는 이를 통해 대기 수요를 줄임으로써 성인 발달장애인이 낮 동안에 또래와 다양한 활동에 참여해 의미 있는 낮을 보낼 것으로 기대했다.

이와 함께 정부는 돌봄 필요성이 큰 최중증 발달장애인 대상 통합돌봄 서비스는 올해 2천340명에서 내년 2천760명으로 늘리고, 최중증인 이들을 위한 긴급 돌봄 제공기관은 현재 2곳에서 내년 5곳으로 확충한다.

최중증 발달장애인 통합돌봄 서비스는 주간 개별 일대일 지원으로 전문인력 지원을 통한 개별화된 개입과 이용자 특성에 맞는 맞춤형(일대일 혹은 그룹형 일대일) 낮 활동 서비스를 제공한다. 24시간 개별 일대일 지원은 낮에는 체육, 요리, 예술, 외출 등 다양한 활동을 하고 밤에는 별도의 주거공간에서 안전하게 생활할 수 있도록 지원한다. 월요일부터 금요일까지 이용하며, 금요일에는 오후 8시까지 제공된다. 주말에는 가족과 함께 보내도록 한다.

<발달장애인의 자립·주거·일자리 지원을 확대한다>

정부는 부모가 나이 들어 더는 발달장애인 자녀를 돌보기 어려운 경우 해당 자녀의 자립 지원을 늘릴 방침이다. 2027년 3월 장애인지역사회자립법 시행에 맞춰 자립지원 시범사업을 본사업으로 전환하고, 자립을 바라는 장애인의 개별 특성을 고려해 주거·일자리·활동 지원 등을 연계할 계획이다.

복지부에 따르면 그간 시범사업 누적 지원자는 올해 6월 현재 615명으로, 이 가운데 84.9%가 발달장애인이었다. 이들의 자립을 위해 내년에는 모든 광역지방정부가, 2030년에는 모든 기초지방정부가 이 사업에 참여한다. 정부는 또 부모의 부재로 자립이 어려운 최중증 발달장애인을 위해 현재 평일에만 운영하는 24시간 일대일 지원 서비스를 주말까지 확대한다.

발달장애인의 일할 능력을 키우기 위해 재직자 훈련 지원 대상을 올해 400명에서 내년 720명으로 늘리고, 같은 기간 중증 장애인 근로 지원인을 1만1천700명에서 1만2천200명으로 확충한다.

 

<발달장애인에 대한 통합 지원을 강화한다>

정부는 발달장애인이 권리와 재산을 안전하게 보호받을 수 있도록 공공후견인 제도와 재산관리 지원 서비스도 강화한다. 또한 16개 시·도에 지역장애아동지원센터를 새로 설치해 장애를 더 일찍 발견하고, 지역사회 복지·교육 자원 등을 연계해 아동에게 필요한 공적 지원을 안내받게 할 계획이다. 발달장애인 특화 정보 통합 애플리케이션(앱)을 구축해 지역 내 기관·단체 검색, 실시간 소통 등 필요한 정보를 더 쉽게 얻도록 지원한다.

발달장애인을 돌봐줄 종사자의 경우 2027년에 가산 수당을 신설해 영아에는 시간당 2천원, 유아에는 1천원을 추가로 지급한다. 신체·정신적으로 고강도 돌봄이 필요한 최중증 발달장애인 돌봄 종사자에게는 전문 수당(현재 월 20만원)을 인상하는 방안도 검토한다.

또 장기간 돌봄으로 심리·정서적 어려움을 겪는 부모와 가족을 위해 가족 휴식 지원 대상을 올해 1만6천명에서 내년 1만8천명으로 늘리고, 부모 교육·상담 지원도 강화한다. 발달장애인 돌봄 국가책임제가 잘 정착되어 장애인과 가족 그리고 관련 근무자들이 더욱 행복해지길 기대한다.

Strengthen the state's responsibility for the care of people with developmental disabilities

The government announced that it will strengthen the state’s responsible care for people with developmental disabilities so that they can live safely without their parents.

 

The late writer Jeon Gyeong-cheol, who called children with severe autism “Peter Pan,” was deeply worried until the very end as he faced death about how to care for his son, who would be left alone after he was gone. As the story of how he visited more than 1,000 disability welfare facilities for his child and was turned away became known, public interest grew even stronger.

In the case of people with developmental disabilities, their cognitive abilities are often relatively impaired and communication is frequently difficult, so they face gaps in care and survival when their primary caregivers, such as parents, are absent. Parents of children with developmental disabilities worry about who will take care of them after they die, and hope to live one more day than their children.

In many countries, care and education for children and adolescents end around the age of 18. However, caregiving for people with developmental disabilities does not end when they become adults. The government has decided to strengthen support tailored to each stage of life, from early childhood to adulthood, to reduce care gaps for people with developmental disabilities.

 

The Ministry of Health and Welfare, together with related ministries, announced a plan to promote the “National Care Responsibility System for People with Developmental Disabilities,” one of the national policy tasks. People with intellectual and autism spectrum disabilities, among others with developmental disabilities, require everyday support due to cognitive and communication limitations as well as challenging behaviors such as self-harm and harm to others. The proportion of people with disabilities who can live independently in daily life is 64.8% overall, but only 26.4% among people with developmental disabilities.

The number of people with developmental disabilities increased from 248,000 in 2020 to 288,000 in 2025, and their proportion of all people with disabilities also rose from 9.4% to 11.0%. In 2025, 69.5% of people with developmental disabilities were adults (ages 18–64), and 6.0% were 65 or older. This is different from the aging of the population, where the proportion of registered people with disabilities in our country aged 65 or older increased from 49.9% in 2020 to 56.9% in 2025. The average age at the time of death of people with developmental disabilities was 57.2 years, which is 21.1 years younger than the average age at death of all people with disabilities (78.3 years).

Therefore, the state's support for people with developmental disabilities should be provided intensively over their short lives. As living standards and medical development advance, the lifespan of people with developmental disabilities is gradually increasing, so attention should be developed toward middle-aged and older adults. Considering the average annual growth rate over the past five years (about 3.1%), the government expects the number of people with developmental disabilities to increase to 335,000 in 2030, accounting for 12.8% of the total. Applying the adult ratio as of 2025, the number of adults with developmental disabilities will increase to about 233,000 by 2030.

 

The government has prepared a variety of measures to support people with disabilities in becoming independent through the National Care Responsibility System for people with developmental disabilities. The Ministry of Health and Welfare’s budget for support of people with developmental disabilities increased from 4 billion won in 2015 to 488 billion won in 2026, about 122 times higher, and the government’s 2027 proposal allocates 572.1 billion won. In 2014, the Act on the Guarantee of Rights and Support for Persons with Developmental Disabilities was enacted, and the related budget increased significantly.

Minister of Health and Welfare Jeong Eun-kyeong said, “The inter-ministerial budget for 2027 has been set to increase by 15.9% to about 2.8 trillion won,” adding, “By 2030, the number of people with developmental disabilities will increase further, so securing an appropriate amount of services will be necessary, and we will secure the budget every year.”

The government will first increase the hours families with disabled children can use childcare support services by 10%, from 1,200 hours per year in 2026 to 1,320 hours in 2027, to ease the childcare burden. The number of specialized and inclusive daycare centers for children with disabilities, currently at 2,008, will also increase by 80 next year. For early intervention for children with disabilities and related reasons, the number of people eligible for developmental rehabilitation services will increase from 110,000 this year to 116,000 next year, and the number of people eligible for after-school activity services will increase from 11,500 to 13,000 during the same period. Increasing the time and budget for social support for children with disabilities could reduce the caregiving burden on parents and others.

 

The government plans to expand the number of people eligible for daytime activity services for adults with developmental disabilities from the current 15,000 to 20,000 next year, and to 30,000 by 2030. Through this, the government hoped that by reducing waiting demand, adult people with developmental disabilities would be able to spend meaningful days during the daytime by participating in various activities with their peers.

In addition, the government will increase the number of people receiving integrated care services for people with severe developmental disabilities who require high levels of care from this year’s 2,340 to next year’s 2,760, and the number of emergency care providers for these most severely disabled individuals will be expanded from the current two to five next year.

The integrated care service for individuals with severe developmental disabilities provides individualized interventions through specialized personnel support in one-on-one daytime support, along with customized daytime activity services tailored to the user’s characteristics (either one-on-one or group-based one-on-one). The 24-hour individual one-on-one support helps them engage in various activities during the day—such as physical education, cooking, the arts, and going out—and live safely at night in a separate residential space. It is available Monday through Friday, and on Fridays it is served until 8 p.m. Make sure to spend the weekend with your family.

The government plans to increase support for the independence of children whose parents are elderly and unable to care for them due to developmental disabilities. In line with the enforcement of the Act on Persons with Disabilities’ Community-Based Independence in March 2027, the pilot independence support project will be converted into the main project, and support for housing, employment, and activities will be linked according to the individual characteristics of persons with disabilities who wish to become independent.

According to the Ministry of Health and Welfare, the cumulative number of participants in the pilot project has reached 615 as of June this year, and among them, 84.9% were people with developmental disabilities. To support their independence, all metropolitan local governments will participate in this project next year, and all basic local governments will participate in 2030. The government will also extend the current 24-hour one-on-one support service, which is currently available only on weekdays, to include weekends for people with severe developmental disabilities who have difficulty becoming independent due to the absence of their parents.

To enhance the work capabilities of people with developmental disabilities, the number of trainees supported by current employees will increase from 400 this year to 720 next year, and during the same period, the number of workers with severe disabilities receiving employment support will be expanded from 11,700 to 12,200.

 

The government will also strengthen the public guardian system and property management support services so that people with developmental disabilities can have their rights and property protected safely. In addition, new Regional Support Centers for Children with Disabilities will be established in 16 cities and provinces to detect disabilities earlier and connect children with the necessary public support by linking community welfare and educational resources. An integrated information app specialized for people with developmental disabilities will be developed to help them access necessary information more easily, such as searching for local organizations and groups and communicating in real time.

For workers caring for people with developmental disabilities, an additional allowance will be introduced in 2027, providing an extra 2,000 won per hour for infants and 1,000 won per hour for young children. They are also considering raising the professional allowance (currently 200,000 won per month) for caregivers of people with severe developmental disabilities who require high-intensity physical and mental care.

In addition, to support parents and families experiencing psychological and emotional difficulties due to long-term caregiving, the number of people eligible for family rest support will increase from 16,000 this year to 18,000 next year, and support for parent education and counseling will also be strengthened. We hope that the National Care Responsibility System for people with developmental disabilities will be firmly established, so that people with disabilities, their families, and related workers can live happier lives.
